So, on this table right, what type of dice do I roll? Because it's not very clear on the table, in the rules text (on p22) or in the extended example (on p23)?

Is there an official errata for this book, because I haven't been able to find it?

It's an extension of the core rules Fear Table, so it uses a d20.

Nope. Other than the confusion on which die to use on this table (which has only come up once before), we haven't seen any big feedback on "errata" for this book.
