I think the only difference on the character sheets would be dropping Guts from preprinted skills lists. I may well have forgotten something, but I really do think that's it.

I made this one a while ago along with some matching NPC cards.
The character sheet is a landscape tri-fold like an old travel passport.

Great sheet Vinzent! Folded on the tri-fold wear marks, brochure style, it slips beautifully in an Explorer-sized book.
