Do you factor in situational combat rules if that maneuver is your typical attack?

For example if my character's typical attack is Full Auto with an AK-47, do you triple the damage when factoring combat rating?

Or if his typical attack is to use an Off-Hand attack do you add the damage from both hands when factoring combat rating?

Nope. It's just a single attack not multiple ones for any reason. That's why the Bolt example just lists the damage for a single bolt.

Combat Ratings are a "baseline" for each side. Things like autofire and offhand attacks are options open to both sides, so typically they would balance each other out (though if a character had Edges that improved those options of course they are taken into account).

If one side doesn't have access to such, then that falls under, "...make sure to also think about additional advantages either side might have..." but also keeping in mind that some of those "advantages" have balancing factors to their effects (autofire for instance is less likely to hit, typically uses triple the ammunition per action, has an increased chance of Innocent Bystander, and a higher chance of a critical failure).
