This topic came up yesterday.

If a character has taken a wound and wants to patch himself up what are the penalties to his roll?
    -1 (-1 for his wound, being the patient)
    OR
    -2 (-1 for his wound PLUS -1 as he is also the doctor)

The latter. Check pg. 76 (SWD) under Modifiers for healing <italics added>, "A wounded character trying to heal his own injuries suffers from both effects (his wounds plus the wound penalty to the Healing roll)."
