1. Does the parry bonus from these two items stack? In this case a +3 Parry, at the same time taking a -4 for using a two-handed weapon one handedly.

2. Can I Shield Bash with my left arm, or does that even matter? The rules are pretty clear that it is not an attack roll.

3. Can I use my large shield (picture the Spartans in 300) to attack people with a Fighting roll?

As long as he meets the minimum Strength for the spear, sure.

The books states, "Actions that require precise eye-hand coordination, such as Fighting or Shooting, suffer a –2 penalty when done with the off-hand."

"Officially," it's up to the GM whether the Push manuever (or anything other than Fighting or Shooting) requires "precise eye-hand coordination" and would be subject to the penalty.

Sure, it would just be using the shield as an Improvised Weapon, meaning using it as a weapon reduces Parry by 1. And since that would be a Fighting roll, the offhand penalty would definitely apply.
