The rulebook (p.14) states that "during combat, characters ascend at half their Strength per round if using ropes or with decent hand- or footholds".

What about outside of combat? Is the rate Strength (in inches) or Climbing die type (in inches). What about if there are no decent hand- or footholds?

Outside of combat, it doesn't really matter. You use the basic rules and the characters make a Climbing roll for every 10". If time is an issue, then combat rounds apply. And if time isn't an issue, it doesn't matter.
