I almost put this as a NE question, because of the strength needed to pull this off on a regular basis, but really it's a general question.

Can you grab/grapple someone and throw them on the same turn by taking a MAP?

If so, what could the grabee do to prevent it (other than resisting the opposed roll or having a held action to break free)?

I assume you'd use the size/weight of the throwee to base damage to the target on, but what about the damage to the throwee? When would it do heavy damage (either to throwee or to the target)?

Typically, no, you couldn't perform both actions in the same turn because it represents using the same hands. Actions that require the hands are limited by the hands.

As far as damage, I'd just apply the same damage to both. Heavy damage would be dependent on whether the other object has Heavy Armor or not.

Also, since another character is not an inanimate object, I'd probably apply at least a -2 penalty to the Throwing roll.
